Appointments v2 CSV File Format

To use the Updox v2 upload for patient information, the CSV file must contain the following columns.  It's OK if some fields are blank but the column must still be present.

•Practice ID

•Practice patient ID

•Practice provider ID

•Provider first name

•Provider last name

•Appointment unique ID (if available)

•Appointment date

•Appointment time
