Blast Patient List CSV File Format

To use the Updox Standard Upload for Blast patient lists, the CSV file must contain the following columns (use the exact column name and column order).  It's OK if some fields are blank but the column must still be present.

•Patient ID

•First Name

•Last Name

•DOB

•Gender

•Address 1

•Address 2

•City

•State

•Zip Code

•Email

•Home Phone

•Mobile Phone

•Active

For the State column, use the two character state abbreviation.

For the Active column, enter TRUE if the record is active.
