Contact CSV File Format

To use the Updox Standard Upload for address book patient contact information, the CSV file must contain the following columns (use the exact column name and column order).  It's OK if some fields are blank but the column must still be present.

•Patient ID

•First Name

•Last Name

•Business Name

•Address 1

•Address 2

•City

•State

•Zip Code

•Email

•Mobile Phone

•Active

•Work Phone

•Fax Number

•EHR Account ID

•EHR Contact Location ID

•DOB
