Patient CSV File Format

To use the Updox Standard Upload for patient information, the CSV file must contain the following columns (use the exact column name and column order).  It's OK if some fields are blank but the column must still be present.

•Patient ID

•First Name

•Last Name

•DOB

•Gender

•Address 1

•Address 2

•City

•State

•Zip Code

•Email

•Home Phone

•Mobile Phone

•Active
