Patient Portal

Location: Address Book

 

To manually create a patient portal account:

1.Go to Address Book.

2.Enter patient's name in the Search box.

3.Select the patient from the search results list.

4.Click Patient Portal tab.

5.Click CREATE ACCOUNT.

 

 

To modify a patient portal account:

1.Click Patient Portal tab.

2.To print a card with the patient's username and password, click PRINT CARD.

3.To change a patient's username, click EDIT USERNAME then enter the changes.

4.To change a patient's password, click RESET PASSWORD.

5.To re-invite the patient to the Patient Portal, click RE-INVITE.  (If the RE-INVITE button isn't present verify the patient's email address is on file.)

6.If the patient doesn't want to receive secure messages and their healthcare records in the Patient Portal, click OPT OUT. If they later decide they want to opt in they can do so by signing into the Patient Portal. The following message displays. Click Yes... to continue or No... to cancel.

 

 

 

Selecting any of these patient portal actions requires the patient to change their password the next time they sign into the portal.
