Appointments CSV File Format

To use the Updox Standard Upload for patient appointment information, the CSV file must contain the following columns (use the exact column name and column order).  It's OK if some fields are blank but the column must still be present.

•PATIENT ID

•PROVIDER FIRST NAME

•PROVIDER LAST NAME

•PROVIDER ID

•APPOINTMENT ID

•APPOINTMENT DATE

•APPOINTMENT TIME

•APPOINTMENT DURATION

•APPOINTMENT SUMMARY

•APPOINTMENT ACTIVE

•APPOINTMENT STATUS
